Every documented change should state why it was made. For individuals who are unfamiliar with the circumstances prior to the change, the rationale can explain the justification. What is impacted by the change is listed in the scope of the change documents. The most important part—the approval—documents the management's support of the change. It is not essential to document which applications are not impacted by the change.
